Personal Information Collection Statement

HKMU Physiotherapy Centre (the “Centre”) is committed to protecting the privacy and security of your personal information. This Personal Information Collection Statement explains how the Centre collects, uses, and safeguards the personal data of its patients.

1. Collection of Personal Information
The Centre collects personal data that is necessary for providing physiotherapy services to you. This may include: Your name, gender, contact details, date of birth, identity document type and number, medical history, and relevant personal data required for your physiotherapy assessment and treatment.

The provision of your personal data is voluntary. However, if you do not provide the requested information, the Centre may not be able to provide you with appropriate physiotherapy services.

2. Use of Personal Information
The Centre uses your personal data to:
(i) Verify your identity;
(ii) Assess your medical conditions;
(iii) Provide treatment / services;
(iv) Maintain clinical records and reports;
(v) Communicate with you in relation to arrangement / provision of services;
(vi) Process payments (e.g. credit card or electronic means) and / or insurance claims from you;
(vii) Conduct activities for teaching / education purposes (e.g. clinical practicum/ internship for physiotherapy students, if any);
(viii) Operate and maintain information systems and/or mobile applications for patients and therapists;
(ix) Evaluate and improve our services;
(x) Provide promotional and direct marketing materials (services, or health related events /activities) to you after receiving your consent in accordance with the Personal Data (Privacy) Ordinance.

3. Disclosure of Personal Information
The Centre will keep your personal data confidential and only disclose it when required by law, for your treatment, or with your consent. In addition to our staff for the purposes stated in this statement, we may make your personal data available to the following parties (whether within or outside Hong Kong):
(i) Doctors / Healthcare providers for purposes relating to your healthcare and physiotherapy services;
(ii) Your insurer or its agents or broker and/or our bank(s) and/or payment service provider(s);
(iii) Our service providers who provide to us services in connection with our services, operations and/or business (including but not limited to clinical, laboratory or technical services, billing and accounting, information technology support);
(iv) Academics/ students/ trainees from hospitals, healthcare or educational institutions who are authorized by us for teaching / education purposes.

The Centre will retain your personal data for the duration necessary to fulfill the purposes outlined in this statement, or as required by applicable laws and regulations.

4. Your Rights
You have the right to request access to and correction of your personal data. You have to make your request for access / correction of your personal data in writing to our Data Protection Officer by email at dpo@hkmu.edu.hk. Any data access request is subject to a fee to cover related administrative costs.

This Personal Information Collection Statement may be updated from time to time. The latest version will be available on the Centre’s website.
